Birth Certificate & Social

Once your baby is born, your midwife will file a birth certificate within 5 days after birth. She will bring a paper copy to your 48-hour postpartum visit for you to review and sign. Once it is filed, the birth certificate will be ready within about 2 weeks.

A social security number is requested by you, the parents, when your midwife checks the box labeled “social security number requested for the child” on the birth certificate. If you request a social security number for your child with the birth certificate, a social security card will be automatically issued to your baby and mailed to the address given on the birth certificate. When you receive this card, that is a good indication that your birth certificate is ready!

To order the birth certificate, go to the Birth Record Application, fill out the form, print, and mail with payment and a copy of your ID to the Vital Statistics office. Instructions and prices are provided on the form.

You may also visit your local Vital Statistics office, fill out the form there, provide an ID, and they will print your certificate while you wait.
